This session will discuss the value and benefits of collaboration between health care providers, social service providers, legal service providers, and other entities with the goal of providing holistic and person-centered care to individuals with disabilities or chronic health conditions to help improve their health outcomes.

Workshop 2 — Services Available to Individuals with Intellectual and/or Developmental Disabilities
This workshop will provide an overview of the supportive services available to individuals with intellectual and/or developmental disabilities. A brief discussion of the guardianship process and supported decision making will also be included.

Workshop 3 — Jail Diversion for Individuals with Mental Illness
This workshop will discuss state initiatives and supportive services for individuals with mental illness involved in the criminal justice system to help divert them from incarceration to treatment and recovery.
